We are seeking an enthusiastic and dedicated
Registered Nurse (RN) for our Primary Care team. In this role, you will provide exceptional nursing care to patients in a primary care setting, focusing on health education, patient advocacy, and the management of chronic and acute illnesses. You will work closely with a multidisciplinary team to deliver patient-centered care to improve overall health outcomes.
Key Responsibilities:
- Conduct thorough patient assessments and triage patients efficiently
- Develop and implement individualized care plans in collaboration with the healthcare team
- Provide health education to patients and their families regarding chronic disease management, medication adherence, and preventive care
- Perform routine nursing procedures such as administering medications, injections, and immunizations, as well as assisting with minor procedures
- Monitor and document patient responses and progress in electronic health records
- Coordinate follow-up appointments, referrals, and community resources for patients as needed
- Participate in quality improvement initiatives to enhance patient care standards
- Work collaboratively to ensure continuity of care and promote a healthy lifestyle among patients
Requirements
- Active and unrestricted RN license in the applicable state
- Associate's Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
- 3+ years of experience in a primary care or outpatient clinic setting preferred
- Current CPR/BLS certification (AHA preferred)
- Strong communication, organizational, and patient advocacy skills
- Ability to work in a team-oriented environment and adapt to a dynamic healthcare setting
- Proficiency in using electronic health record systems and technology for patient management
- Must pass a government background check
- Able to provide 3 professional references
