Position Summary
The
Nurse Care Manager (NCM) Lead serves as a frontline leader and resource for Bloom Healthcare’s nursing care management team. This role provides day-to-day guidance, operational support, and mentorship to ensure high-quality, coordinated care for our patients. Working closely with the Clinical Support Manager, the NCM Lead helps drive team performance, supports ongoing education and process improvement, and fosters a positive, engaged team culture.
Key Responsibilities
Team Leadership & Support
- Serve as the first point of contact for daily team questions and workflow support
- Lead or co-lead weekly NCM team meetings in collaboration with the Clinical Support Manager
- Conduct monthly one-on-one check-ins with team members and assist with ongoing coaching and development
- Collaborate on annual performance reviews, including preparation, documentation, and conducting review discussions
- Partner with leadership to provide input and updates during monthly support leadership meetings
- Coordinate and maintain coverage plans and recurring schedules to ensure adequate staffing and continuity of care
- Review and approve PTO requests and timecards in Insperity on a bi-weekly basis
Onboarding & Training
- Manages onboarding new team members, including schedule creation, orientation, and initial training
- Collaborate with the Clinical Support Manager to develop meeting and training agendas
- Lead and/or coordinate team education sessions and ongoing training initiatives
Quality & Performance
- Track, maintain, and analyze team metrics to monitor performance and identify opportunities for improvement
- Support care plan reviews, task management, and other quality audits as part of team coaching
- Participate in bi-weekly or monthly meetings with the Clinical Support Manager to review team progress and align on priorities
- Help design and refine workflow systems that promote efficiency, consistency, and excellent patient outcomes
Qualifications
- Education & Licensure: Active, LPN or RN license required
- Experience: Minimum 3 years of nursing experience in primary care, home health, or care management; prior leadership or mentorship experience preferred
- Skills:
- Strong leadership and communication abilities
- Excellent organizational skills and attention to detail
- Ability to balance coaching with accountability
- Proficient in EHR systems, Microsoft Office Suite, and reporting tools

The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically
falls between LPN $28-32 hourly / RN $39-44 hourly with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
