GHR Healthcare is currently hiring a skilled acute care RN for a full-time, 13-Week contract opportunity with an incredible Hospital at Home program near Paramus, NJ!
- Schedule: Monday-Friday, Weekend Rotation
- Shift: 4(10) Hour Shifts Per Week - Varying Start Times

In this vital role, you will:
- Perform comprehensive nursing assessments, monitoring, evaluations, interventions, and treatments for acute care patients in their homes.
- Apply advanced nursing knowledge and critical thinking to identify, prevent, and resolve complex acute patient care issues.
- Collaborate effectively with patients, families, and the interdisciplinary team, including therapists and specialists, to ensure seamless care coordination.
- Incorporate social determinants of health into individualized care plans.
- Assist with biometric monitoring technology and respond to vital sign data.
- Provide tailored patient education on disease and symptom management, medication, treatments, and procedures.
- Communicate pertinent assessment findings to remote providers and multidisciplinary team members.
- Coordinate patient discharge from the hospital-at-home program, ensuring smooth transitions to primary care, home care services, and specialists.
- Implement all care plan interventions with a patient-centered approach.
- Demonstrate competence in a range of procedural skills including, but not limited to: phlebotomy, vascular access device management, hydration therapy, medication administration (various routes), wound management, catheter care, specimen collection, stoma management, and compression therapy.
- Maintain a professional and therapeutic patient-nurse relationship.
- Contribute to program development and quality improvement initiatives for the RN role in home-based nursing care.
- Ensure compliance with all state and federal nursing guidelines and reporting requirements.
Work Schedule & Location:
- Full-time: 40 hours per week (10-hour shifts), including Friday, Saturday, and Sunday every other week.
- Weekend and holiday rotation required.
- Must be located in the New Jersey area to conduct onsite patient visits.
Our Ideal Candidate Brings
- An active New Jersey Registered Nurse (RN) license, or the ability to obtain it by the start date.
- Associate's or Bachelor's Degree in Nursing from an accredited program.
- 2+ years of nursing experience, with a strong preference for acute, ICU, ER, or home health settings.
- Current CPR/BLS certification.
- Valid driver’s license.
- Excellent interpersonal, verbal, written communication, and organizational skills.
- Proficiency in medical terminology, procedures, and treatments.
- High clinical proficiency in nursing.
- Comfortable with technology, including EMR systems and Windows-based computer skills.
- Knowledge of state and federal home health regulations.
- Ability to exercise independent judgment and initiative.
