We are seeking
Nurse Assessors to join the
Independent Assessor (IA) Project, conducting both
in-home and telehealth assessments for consumers applying for community-based long-term care programs.

Key Responsibilities:
- Conduct assessments (telehealth or in-home) using the Uniform Assessment System of New York (UAS-NY) to evaluate consumer health, abilities, and long-term care needs.
- Review medical documentation and referrals to ensure accurate, informed evaluations.
- Maintain precise electronic documentation and properly code all UAS-NY data.
- Educate consumers on healthcare options, managed care plans, and community-based resources.
- Coordinate care and referrals with interdisciplinary teams.
- Collaborate with the Clinical Quality Assurance Department to uphold compliance and quality.
- Stay current on state regulations and best practices in long-term care.
- Manage your own scheduling and consumer appointments to create a balanced, autonomous workflow.
What We're Looking For
Education & Licensure:
- Graduate of an accredited nursing program (BSN preferred).
- Active RN license in the State of New York.
- Valid NY State ID or Driver’s License.
Experience
- 1–2 years of in-home care or community-based nursing preferred.
- Experience in geriatric, home health, or chronic illness care is a plus.
- Familiarity with UAS-NY assessments and related systems is highly desirable.

- Self-motivated and highly organized, able to manage your own time and caseload.
- Proficient in using electronic health records, UAS-NY systems, and telehealth platforms.
- Reliable transportation required for in-person assessments.
