We are hiring
Registered Nurses (RNs)for the
Independent Assessor (IA) Project, conducting
in-home and telehealth assessments for individuals applying for community-based long-term
geriatric care programs.
Location: Manhattan, NY

What You'll Do
- Conduct UAS-NY assessments in-home or via telehealth.
- Review medical records and ensure accurate documentation.
- Educate consumers and coordinate care with providers.
- Manage your own appointments and maintain compliance.
What You Need
- 1–2 years of experience as an RN in one-on-one in-home, community, skilled nursing facility, long term and geriatric care required
- RN license valid in New York
- BSN is required
- Familiarity with Uniform Assessment System (UAS)-NY is a plus
- Strong communication, time management, and Electronic Health Record (EHR) skills
- Reliable transportation for in-person visits
