The Medical Services Patient Intake Coordinator is responsible to support Chapters Palliative Care and Hospice Provider Visits. Works closely with the Medical Services providers and Chapters Billing team to support intake, scheduling, and transition from palliative care to hospice.
Qualifications
- Associate’s degree or equivalent combination of education and experience
- Minimum of two (2) years of related experience
- Experience in Health Information Management (HIM) using electronic health records preferred
- Strong analytical, problem-solving skills, and goal/outcome orientation
- Excellent skills in customer service, written and verbal communication, and critical thinking
- Skilled in computer operations. Proficiency in MS Office software applications (Excel, Word, Outlook), on-line research and proven data entry and navigation skills
- Excellent time management skills with ability to prioritize demands to meet client service standards and deadlines
- Well-developed writing and editing skills
- Excellent organizational skills and detail-oriented

Job Responsibilities
- Develops and maintains provider staffing matrix throughout Chapters Health System to include practitioner location, credentials, and accepted insurance billing.
- Performs intake on new patients referred from hospitals and other facilities
- Creates patient records in the EMR/Billing system in advance of scheduling.
- Ensures document upload, referrals processed, and insurance verified;.
- Schedules patient appointments.
- Performs insurance verification for new patients.
- Identifies opportunities for additional insurance contracts to increase the practice’s network.
- Gathers required patient consents and facility referrals.
- Ensures the accuracy and completeness of medical records in accordance with documentation standards and regulatory requirements.
- Uploads inpatient visit documentation and creates the billing required
- Creates Hospice GIP Patient visits in My Unity. This includes creating the encounter, uploading the hospital visit notes and routing to the provider for review and signoff prior to billing.
- Maintains EMR access at hospitals where Chapters provides inpatient care.
- Provides administrative support to the Manager, Medical Services, on special projects and provides back-up for the Medical Services Scheduling Team.
- Performs other duties as assigned.
