🩺 Registered Nurse Assessor - UAS-NY | Hybrid (Tele + In-Field) | New York State

Licensure Required

Active RN License in New York State

Required Experience

2 years of experience in a home care setting within the last 10 years, including:
- Field work with CHHA, LHCSA, or MLTC
- Conducting OASIS or similar comprehensive assessments
- Primary care for homebound individuals
- Hospice, Assisted Living, or HCBS care management
- LPN/paraprofessional (HHA) experience caring for 2+ patients/week

Preferred Qualifications (Gold Standard)

Experience conducting UAS-NY assessments (roles 40 or 45) within the last 12 months
Geriatric care and chronic disease management in the home setting
Combined RN and prior LPN/HHA home care experience (2+ patients/week)

Does NOT count as Required Experience (only considered toward preferred):

Private duty nursing or CDPAS with just one patient
Utilization review-only case management
Assessments conducted entirely telephonically

📋 Responsibilities Include

Conduct initial in-home pre-evaluations to guide care planning
Review medical documents and referrals
Enter and transmit evaluation data via electronic forms
Document discrepancies or concerns during evaluation
Promote care continuity and reduce fragmentation
Educate consumers on service and waiver options
Communicate with Call Center and consumers for enrollment
Collaborate with interdisciplinary care teams
Record data and case notes in CRM
Promote health education and prevention
Maintain knowledge of community resources and payor rules
Meet all performance standards
Other duties as assigned by leadership

Exclusivity Required: Candidates cannot conduct UAS assessments elsewhere
