Job Summary:
Primary function is to facilitate the referral intake process and coordinate care with the interdisciplinary team, the patient, their family, and the referral source.
Job Qualifications:
Education: High school graduate. Graduate of an accredited school of vocational/practical nursing, preferred.
Licensure: Current driver’s license.
Experience and Skills: Candidates should have at least one year of scheduling or coordinating experience, along with healthcare services for patients by managing nurse visits, tracking patient care, and maintaining operational efficiency. The role involves extensive communication with healthcare staff, therapy providers, and support teams to uphold timely and quality patient care.
Transportation: Reliable transportation and valid auto liability insurance.
Environmental and Working Conditions: This position is set in a professional office environment with an open layout and a team-based dynamic. Work is primarily in-office, though remote work may be possible at the discretion of management.
Physical and Mental Effort: Sedentary Position, with ability to lift 25 pounds occasionally.
