Summary
The Clinical Intake Nurse serves as the first point of clinical contact for patients entering home care services. This role is responsible for assessing patient eligibility, coordinating intake processes, and ensuring smooth transitions into home care programs. The nurse acts as a liaison between patients, families, physicians, and the home care team to deliver compassionate, efficient, and compliant care.
Responsibilities
(50%)* Conduct initial clinical assessments of patients referred to home care, including medical history, current health status, and care needs. Review physician orders and referral documentation to determine appropriateness for home care services.
(25%)* Coordinate intake process by gathering necessary consents, insurance information, and verifying eligibility. Educate patients and families on home care services, expectations, and available resources. Communicate effectively with referral sources, physicians, and internal staff to facilitate timely admissions.
(25%)* Ensure compliance with regulatory requirements, agency policies, and documentation standards. Oasis Transfers. Support quality improvement initiatives by identifying gaps in intake processes and recommending solutions.
OTHER DUTIES AND RESPONSIBILITIES:
Ensures that orders and all necessary information to initiate services are obtained.
Responds to inquiries and requests for client services, providing information regarding agency and community services available.
Performs pre-admission calls.
Receives and processes physician orders.
Communicates patient needs with agency personnel; changes in orders.
Triage patient calls to appropriate agency personnel.
Utilizes Agency documentation system to record services provided according to agency, professional, and program standards.
Assists in maintenance of patient records.
Educates external health care professionals regarding range of services agency provides. Assists in Agency community education activities as requested.
Participates in Agency quality and performance improvement process.
Participates in Agency projects and committees as assigned.
Participates in weekend and holiday rotations and on-call duties as identified in personnel policies.
Participates in staff meetings, in-service education programs, and other opportunities for learning.
Performs other duties as assigned.

PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS:
Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.
Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.

Overall vision and hearing are necessary with or without assisted device(s).
Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching.
Some exposure to blood borne pathogens and other potentially infectious material. Must follow MyMichigan Health blood borne pathogen and TB testing as required.
Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.
Overall dexterity is required including handling, reaching, grasping and feeling. May require repetition of these movements on a regular to frequent basis.
