Elevate Care is seeking a reliable and detail-oriented Patient Appointment Coordinator to join our team. This key role ensures residents receive timely medical care by managing and scheduling all patient appointments with physicians, specialists, and other healthcare providers.
Responsibilities
- Schedule and confirm resident medical appointments with providers inside and outside the facility.
- Coordinate transportation and communicate appointment details to residents, families, and staff.
- Maintain an accurate calendar of appointments and follow up on changes or cancellations.
- Ensure all necessary documentation is prepared and sent with residents for appointments.
- Work closely with nursing staff, physicians, and families to support continuity of care.
- Keep records updated in compliance with facility policies and healthcare regulations.
Qualifications
- Previous scheduling or administrative experience in healthcare or a nursing home setting preferred.
- Strong organizational and multitasking skills.
- Excellent communication and customer service abilities.
- Proficiency with scheduling systems and Microsoft Office a plus.
- Compassion and dedication to serving residents with dignity and respect.
