We are looking for a
Director, Patient Care to lead, scale, and evolve our Care Navigation and Clinical teams. This role plays a pivotal part in ensuring that every patient supported by Carrum Health receives world-class, compassionate, and coordinated care. You will be responsible for operational performance, clinical quality, and patient experience across the care continuum.
Reporting to the Vice President, Patient Care, you will provide direct leadership over Care Navigation and Clinical Services, ensuring alignment, collaboration, and consistency in care delivery. You'll set the strategic direction for the Patient Care function, balancing clinical integrity with operational excellence to deliver an unmatched patient experience.
The salary range for this role is $130,000–$160,000, depending on experience and location.This is a full-time, remote position eligible for full benefits and bonus. We are looking for candidates willing to work on a Central or Pacific Time schedule.

- Lead and develop a high-performing, multidisciplinary Patient Care organization that includes Care Navigators and clinical team members.
- Establish and execute a strategic vision for patient care delivery that ensures clinical quality, service excellence, and scalability as Carrum grows.
- Oversee daily operations of the Care Navigation and Clinical teams, ensuring workflows, communication, and patient touchpoints are seamless and effective.
- Partner closely with clinical leadership to ensure quality standards, protocols, and clinical pathways are followed consistently.
- Ensure a differentiated, high-touch experience for every patient, combining empathetic support with operational precision and clinical expertise.
- Collaborate with all cross-functional leaders to enhance patient engagement, streamline workflows, and identify opportunities for innovation.
- Define, monitor, and analyze key performance metrics, including patient satisfaction, clinical outcomes, engagement rates, and care utilization.
- Partner with leadership to design and implement technology and process improvements that optimize care coordination, clinical performance, and patient outcomes.
- Lead workforce planning, staffing, and professional development across both clinical and non-clinical roles to ensure adequate capacity and readiness.
- Serve as a visible, hands-on leader who inspires excellence, accountability, and collaboration within a remote team environment.
- Manage budgets and resource allocation for the Care Navigation and Clinical functions, ensuring efficiency and sustainable growth.
- Continuously identify opportunities to innovate, scale, and elevate the patient experience using data, technology, and clinical insights.
We're excited about you because…
- You have 10+ years of experience in patient operations, care coordination, or healthcare service delivery roles, including 5-8 years of leadership experience managing leaders of leaders as well as clinical or multidisciplinary teams.
- You are a clinically grounded leader with an active RN license (or other clinical licensure such as NP, PA, or LCSW) is strongly preferred.
- You have proven success leading both clinical and non-clinical teams, balancing patient experience, operational performance, and care quality.
- You are a strategic operator who can set vision, manage execution, and adapt to the needs of a growing organization.
- You are data-driven, skilled at interpreting metrics and using insights to improve outcomes, quality, and operational efficiency.
- You are passionate about building and developing people, fostering an environment of empathy, learning, and accountability.
- You have exceptional communication and collaboration skills, able to engage effectively with clinicians, executives, and partners.
- You handle confidential information appropriately and maintain full compliance with HIPAA and other regulatory standards.
- You embody "patient first" values and are deeply motivated by the opportunity to improve the patient journey through high-quality, coordinated care.
- You thrive in a fast-paced, evolving environment, comfortable with ambiguity and inspired by mission-driven work.
- Experience with self-funded employer benefit plans, bundled payment models, or Centers of Excellence programs is a plus.
