Job Description
The MDS nurse is responsible for conducting comprehensive assessments of residents' health status, coordinating the completion of the MDS assessments, and ensuring accurate and timely documentation. This nurse plays a key role in care planning by collaborating with interdisciplinary teams to develop and update individualized care plans based on MDS findings. They also help ensure compliance with federal and state regulations related to resident assessments and care delivery. The MDS Nurse monitors resident progress, addresses any concerns, and supports quality improvement initiatives within the facility.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Complete and submit accurate MDS assessments in accordance with federal and state regulations and facility guidelines
- Coordinate interdisciplinary care planning based on resident assessments and care needs
- Monitor and ensure compliance with RAI processes and timeliness
- Collaborate with clinical team to gather assessment data and ensure documentation supports coding
- Identify changes in resident condition that require assessment updates or care plan revisions.
- Participate in quality assurance, audits, and survey preparedness activities related to resident assessments.
- Educate nursing staff on documentation practices that support MDS accuracy and reimbursement compliance.
Required Skills/Abilities
- Current state licensure required
Education And Experience
- LVN or RN
- Current state licensure required.
Physical Requirements
- Prolonged periods of sitting at a desk and working on a computer.
