As an HHA Coordinator, you will serve as key support for the Home Care Operations team, managing the HHA staffing systems and assigning staff to patient cases.

Position Responsibilities
- Maintain HHAeXchange (home health aide vendor) system: broadcast HHA cases for staffing, update HHAX with patient demographics, authorization, and recertification information
- Collaborate with lhcsa (licensed home care agency) coordinators to ensure scheduling, billing, and authorizations are accurate.
- Collaborate with office/field staff with any changes to HHA patient's care
- Troubleshoot lhcsa vendor issues, manage HHAX communications and update HHAX with provider rate increases
- Notify lhcsa vendor of issues regarding aide services.
- Process un-billing requests
- Run weekly expiring authorization reports to ensure services continue in accordance with the POC and NYS Department of Health regulations.
- Maintain Careficient (patient care) software system: assign vendors to a patient schedule, collaborate with the nursing supervisor and nursing staff to ensure accurate information reflected on 485, HHA POC, and care needs
- Run missed visit reports, remove missed visits from patient's schedule, scan and upload missed visit notes CE
- Send weekly expiring authorization emails to intake staff and nursing staff.
- Assists with staffing patient visits to subcontracted agencies (PT/OT/ST), including sending/retrieving all required documentation for patient management
Qualifications
- High school diploma; college degree preferred
- Excellent communication and customer service skills, both written and verbal
- Tech savvy, with proficiency in Microsoft Office, including Excel
- Prior experience with HHAeXchange and Careficient a plus
- Absolute integrity and sufficient maturity to handle confidential aspects of work
Salary: $21.97 - $24.72 per year
