Position Overview:
Grace Healthcare Services is seeking a compassionate and skilled Admissions Registered Nurse to join our team in Middlesex County, NJ. This role plays a critical part in assessing and addressing the physical, psychological, social, and spiritual needs of patients and their families. The RN develops and implements a comprehensive, responsive plan of care that ensures the highest quality of service.

Key Responsibilities:
Deliver skilled nursing care in accordance with the established plan of care
Coordinate care and services for patients and families, including collaboration with the Interdisciplinary Team (IDT)
Educate patients, families, and caregivers on appropriate care techniques
Act as an advocate for the patient and family through regular communication with attending physicians, facility staff, and care managers
Qualifications:
Active New Jersey Registered Nurse license
Minimum of two (2) years of experience in an acute care hospital (medical, surgical, oncology, or equivalent experience)
Demonstrated clinical competence and commitment to patient-centered care
Excellent interpersonal and communication skills with a compassionate bedside manner
Valid driver's license, reliable transportation, and willingness to travel within your territory
