AuthoraCare Collective is currently seeking an
RN Hospital Liaison Float for our part-time weekend day position supporting our Burlington and Greensboro campuses and service areas. Required orientation and training: 3 days New Employee & Clinical Orientation and at least two consecutive weeks and additional flexible weekdays for job related orientation/training.
Flexible Schedule Options
- 8 hour day shifts [16 hours]
- 10 hour day shifts [20 hours]
Job Summary
Evaluate the clinical needs of newly referred and hospitalized hospice patients. Identify and inform patient and family about appropriate resources. Work with the patient/caregiver and appropriate providers to establish patient/caregiver care goals and identify and implement interventions.
With attending physician and selected members of the IDT, ensure that a medical plan of care is established and implemented for the patient. Communicate effectively with other members of the interdisciplinary team and health care providers, as needed.
Community And Hospital Staff Relations
- Collaborates with hospital staff and Palliative Medicine Team to ensure effective and efficient working operations for referrals and current hospice patients.
- Consistently models respect for, and accountability to, the public, patients/families, and co-workers.
- Communication and actions reflect care, courtesy, honesty, and professionalism.
- Facilitate positive relationships with hospital staff, physicians, and community.
- Meet deadlines and respond to requests in a timely and professional manner.
- Meet agency regulatory/licensure requirements that affect job responsibilities.
- Maintains effective relationships and partnerships with community healthcare providers.
- Attends agency and departmental meetings.
- Ensures professional growth and development, incorporates changes/ advances in nursing practice into own practice.
- Volunteers for projects and accepts assignments for projects as follows through.
Requirements
Education/Experience:
- Must be currently licensed as an RN in North Carolina with a minimum of 3-5 years of experience in an acute or home health setting.
- Bachelor's degree in nursing and Hospice experience preferred. RN Certification in Hospice and Palliative Care preferred.
- Understand and be committed to the Hospice and Palliative philosophy of care.
Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
