About the Position:
Community Care is seeking a dedicated RN/LPN to provide private duty, one-on-one care for a 19-year-old male in the Perryopolis area. As part of our trusted team, you'll deliver personalized care while enjoying the support of a family-run agency that has proudly served local communities for more than three decades.
Available Shifts:
Tuesday (Overnight): 10:00 PM – 7:00 AM
Thursday: 8:00 AM – 5:00 PM
Friday: 7:00 AM – 6:00 PM
Pick up one shift or build a consistent weekly schedule.
Responsibilities Include:
G-Tube care and feedings
Medication administration
Assistance with activities of daily living (ADLs)
Routine monitoring and documentation
Providing safe, respectful, and patient-centered care in a home environment

Requirements:
Current RN/LPN license in the state of Pennsylvania
CPR certification
Experience with pediatrics G-Tube care
Reliable, compassionate, and patient-centered
Reliable transportation
