Position Title: Treatment Nurse (LPN or RN)
Position Type: Full-Time
Salary Range: $27.00 to $45.00 (based on licensure & experience)

Position Overview:
We are seeking a dedicated Treatment Nurse (LPN or RN) to join our team at The Grand Rehabilitation and Nursing at Barnwell. In this role, you will provide skilled nursing treatments, monitor resident conditions, and ensure accurate documentation of care. You will play a critical role in supporting resident health, preventing complications, and ensuring that all treatments are completed according to physician orders and facility standards.
Key Responsibilities:
• Provide direct nursing care and perform all physician-ordered treatments and wound care according to clinical protocols.
• Obtain and record vital signs as part of ongoing resident monitoring.
• Complete finger-stick glucose checks and administer insulin per physician orders.
• Assist with weight monitoring and ongoing clinical assessments.
• Recognize and report changes in resident conditions to nursing leadership or physicians.
• Maintain and organize treatment carts and ensure adequate supply levels.
• Participate in care planning and collaborate with the interdisciplinary team to support individualized resident needs.
• Identify residents at risk for skin breakdown and assist with preventative measures.
• Ensure compliance with universal precautions and infection control standards.
• Accurately document all treatments, interventions, and observations in the EMR.
• Participate in morning report, skin rounds, care plan meetings, and quality assurance initiatives.
• Support staff education related to treatments, skin integrity, and infection prevention.
• Perform additional duties as assigned by nursing leadership.
What We’re Looking For:
• Current and unencumbered LPN or RN license in the state of New York.
• Prior experience in long-term care, wound care, or treatment nursing preferred.
• Strong assessment, organizational, and documentation skills.
• Ability to work both independently and collaboratively within a team environment.
• Basic computer skills and comfort with EMR systems.
• Ability to lift, move, and assist residents as required.
