About the Opportunity:
Madison Health and Rehab is currently seeking a dedicated and motivated Unit Manager/ Wound Care Nurse to join our compassionate and growing team in Madison, WI. This is a vital leadership role that will work closely with our outstanding clinical team to ensure the highest standards of infection prevention and resident care.
Job Responsibilities may include but not limited too:
- 
Perform comprehensive skin and wound assessments on residents. 
- 
Develop and implement individualized wound care plans in coordination with the healthcare team. 
- 
Provide direct wound care treatment including cleaning, dressing changes, debridement, and application of advanced wound care products. 
- 
Collaborate with physicians, nurse practitioners, and other clinical team members. 
- 
Monitor wound healing progress and adjust care plans accordingly. 
- 
Educate staff on wound prevention, staging, and treatment best practices. 
- 
Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements. 
- 
Ensure compliance with infection control standards. 
- 
Assist the DON in managing the day-to-day operations of the nursing department. 
- 
Supervise nursing staff, ensure high-quality resident care, and provide clinical guidance as needed. 
- 
Participate in staff hiring, training, evaluations, and disciplinary processes. 
- 
Ensure compliance with federal, state, and local regulations as well as company policies. 
- 
Participate in care planning, quality improvement initiatives, and interdisciplinary team meetings. 
Qualifications:
- 
Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Wisconsin 
- 
Minimum 1 years of nursing experience in a long-term care or skilled nursing facility preferred 
- 
Wound Care Certification preferred or willingness to obtain 
- 
Excellent communication, organizational, and leadership skills 
- 
Ability to work collaboratively and guide a team effectively
