Required Schedule
Monday - Friday 8:30am-5:00pm w/ a minimum of 1 weekend per month
Job Duties
- Provide direct nursing care assigned by and under direct supervision of an RN and is reportable to the RN and DON.
- The mission of the home health team is to meet the unique needs of clients and families with services tailored to assist them in achieving optimal level of function. 
- Be responsible for the preparation of clinical notes for the patient’s clinical record. 
- Report any changes in the patients condition to the RN and document changes in the patient’s clinical record.
- Attend weekly case conference meetings. 
- Participate in any QA programs as needed. 
- Conduct themselves in a polite and cordial manner when rendering patient care and or communicating with other agency staff.
- Employee will follow policies and procedures and will safeguard the privacy of patient protected health information. 
- All other duties as assigned.

Education/Experience
Graduate of an accredited LPN program.
Minimum of one year of experience, preferably in home health or medical/surgical.
Certificates, Licenses, Registrations
Valid Florida Driver’s License.
Current LPN License in State of Florida
Healthcare Provider CPR required upon hire and maintained.
Computer Skills
Knowledge of basic typing skills, data entry and word processing software, database software and general knowledge of e-mail.
Language Skills
Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.
Mathematical Skills
Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
Reasoning Ability
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
