SHIFT/CORE HOURS: Monday-Friday 8:30 AM-5:00 PM or 4(10-hour shifts), day off varies each week.

Position Summary:
The Licensed Practical Nurse (LPN) in the Call Center is responsible for providing high-quality, compassionate care through telephonic nursing support. This role includes answering inbound calls, managing routine and urgent medical inquiries, offering patient education, and coordinating care with healthcare providers, staff, and family members. The LPN will use clinical judgment to support patient needs and facilitate effective communication across the care team. Position requires excellent customer service skills with patients, and their families, other staff, physicians, vendors, and the public.
Key Responsibilities:
- Answer incoming calls from patients, family members, and other healthcare professionals in a timely and professional manner.
- Assisting the Customer Service Representatives with difficult clinical calls
- Review patient symptoms and concerns based on established clinical protocols and guidelines and coordinates patient care with a provider
- Provide appropriate medical advice, instructions, and follow-up plans as delegated by clinical protocols or supervising RNs/physicians.
- Document all interactions and interventions accurately and promptly in the electronic medical record (EMR).
- Coordinate communication between patients, providers, and internal staff to ensure continuity of care.
- Relay test results, appointment instructions, and medication updates as authorized.
- Escalate complex clinical situations to providers as appropriate.
- Handling the Direct in Triage Bin
- Maintain patient confidentiality and comply with HIPAA and other regulatory requirements.
- If available to assist with in office cover when needed.
Qualifications:
- Current and active LPN license in NYS
- Minimum of 1–2 years clinical nursing experience, preferably in outpatient, urgent care, or call center settings
- Excellent communication, listening, and interpersonal skills
- Strong organizational and multitasking abilities
- Proficient with EMR systems and basic computer applications
- BLS certification (Basic Life Support) preferred or required
