Clinical Chart Review Specialist. In this role, you'll play a vital part in ensuring accurate capture of patient information required for quality initiatives, grant reporting, compliance audits, and clinical improvement projects, helping us move forward with purpose and precision. Essential Job Duties
- Manually review patient charts in the EMR to identify and extract data related to screenings, immunizations, chronic disease management, and other quality measures.
- Ensure accuracy and consistency of chart review findings through thorough documentation and cross-checking.
- Provide data abstraction for quality programs such as HRSA, UDS, HEDIS, PCMH, and value-based care initiatives.
- Support audits and accreditation reviews by ensuring chart documentation aligns with compliance standards.
- Collaborate with Quality, Clinical, and Administrative staff to ensure timely and accurate data collection and sharing.
- Provide feedback to providers or care teams on missing documentation and assist with staff education on EMR charting best practices.
- Ensure front desk coverage at the Administration Office as needed, greeting visitors, answering calls, and supporting administrative tasks.

Requirements
Qualifications:
- High School Diploma or equivalent preferred.
- Medical Assistant certification (CMA, RMA, or equivalent) or comparable clinical experience preferred.
- Prior experience with electronic medical record (EMR) systems required.
- 1–2 years of experience in chart review, medical records, or quality data abstraction preferred.
