Overview
The Registered Nurse Case Manager provides comprehensive nursing care and case management for patients receiving hospice and home health services. This position focuses on supporting patients and families through compassionate, evidence-based care, while coordinating with interdisciplinary teams to ensure quality and continuity of services.
Key Responsibilities
- Manage and coordinate care for patients in hospice and home settings
- Develop, implement, and monitor individualized care plans in collaboration with physicians and interdisciplinary team members
- Conduct patient assessments and respond to changes in condition or care needs
- Educate patients and families on health management, medications, and symptom control
- Maintain accurate documentation and charting according to regulatory and organizational standards
- Provide emotional and clinical support to patients and families during end-of-life care
- Participate in team meetings, case reviews, and quality improvement initiatives
- Support and guide other nursing staff as needed
- Perform additional nursing duties as assigned
Schedule and Shift Details
- Monday through Friday, 8 AM to 5 PM, with flexible start and end times
- On-call rotation averaging approximately five shifts per month
- No regular weekend or holiday requirements unless assigned during rotation
Travel
- Local travel required across multiple counties for patient visits and care coordination
Qualifications Required
- Active Registered Nurse (RN) license in Kansas (KS)
- Minimum 1 year of hospice experience
- Minimum 1 year of management or leadership experience preferred
- Experience in home health or community-based care
- Current certifications: Basic Life Support (AHA BLS) and Advanced Cardiac Life Support (AHA ACLS)
- High School Diploma or GED or higher education in nursing
- Valid driver’s license and reliable transportation
Preferred
- Experience in case management or care coordination
- Proficiency in electronic medical record (EMR) systems
