Position Summary:
The Patient Care Coordinator plays a vital part in administering day-to-day operations of the hospice department and coordinates the services of all team members. This position works closely with the Clinical Director to meet and promote the standards of quality and contribute to the total organization and philosophy.
Essential Duties & Responsibilities:
Scheduling out visits requested by Registered Nurses, Social Worker and ChaplainsScheduling out Palliative care visits for the Nurse PractitionerEnsuring staff complete visits in a timely manner.Serving as the scribe and facilitator for interdisciplinary team meetingsGathering necessary information to input a complete referral.Oversees all tasks related to coordination of patient care for Palliative care.Assists with quality assurance and performance improvement responsibilities. Calls patients/ families to schedule admission time. Coordinates with admission nurse or nurse covering. Cooperates with education and research activities as assigned.Runs reports in EMR to ensure compliance and keeping Daily Activity Report updated.Other duties as assigned by the Clinical Director and/or Clinical Manager.
Work Environment
This position will perform most duties in an office environment. While performing some of the duties of this position, the employee will travel by automobile and is exposed to changing weather conditions.
Physical Demands
While performing the duties of this job, the employee is required to see, talk, or hear. The employee is regularly required to stand, walk, sit, reposition patients; use hands and fingers to feel objects, tools, or controls; and reach with hands and arms. The employee frequently lifts and moves up to 50 lbs.

Requirements:
Job Requirements
Graduate of an accredited LPN program and licensed in Florida preferred. Minimum one year nursing experience preferred.Two years home care or hospice experience preferable.Must be proficient in Microsoft Word, Outlook, and Excel.Able to prioritize and manage time effectively and make independent decisions when necessary. Must be able to effectively operate nursing assessment tools and office equipment to include computer and tablet.Current CPR certification.Valid driver’s license and auto insurance.
