Rutgers, The State University of New Jersey, is seeking an LPN for UBHC – Outpatient. The primary purpose of the LPN position is providing services connecting individuals, in the ISC program with a diagnosis of mental health and substance use disorder, to resources that address issues related to social determinants of health through case management, medical referrals, community outreach and education. Maintain community and follow with all providers. Among the key duties of this position is the following: Provides referral services and medical intervention to individuals Promotes health and safety through the provision of education about medication, Medication Assisted Therapy ( MAT ), HIV / STD , physical health promotion, and other medical topics to promote wellness and recovery. Maintains appropriate individual records as mandated by University Behavioral Health Care policy Supports goals and objectives of University Behavioral Health Care Completes all mandatory educational requirements on an annual basis
