We are seeking a dynamic and compassionate Hospital Liaison to serve as the vital link between our organization, hospitals, patients, and their families. The ideal candidate will excel in fostering relationships with hospital staff, ensuring patients are ready and manageable for care, and providing outstanding communication and support to families.
- Key Responsibilities:**
- Act as the first point of contact with patients and their families, providing compassionate communication and guidance.
- Conduct on-site screenings to assess patient readiness and manageability.
- Build and strengthen relationships with hospital staff to ensure seamless coordination of care.
- Serve as the primary liaison between the hospital and our organization, facilitating smooth transitions.
- Qualifications:**
- Must be a licensed nurse.
- Strong familiarity with UAB and **VA hospitals is required.
- Knowledge of Medicare and insurance processes is a significant advantage.
- Excellent interpersonal and communication skills.
- Ability to build and maintain professional relationships.
