Pay Range: $33 - $59/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

Coordinate patient care activities to ensure appropriate utilization of clinical resources.
Collaborate with physicians, nurses, case managers, and other healthcare professionals to develop and implement patient-centered care plans.
Facilitate communication between interdisciplinary teams to improve patient outcomes and satisfaction.
Support discharge planning by identifying post-discharge needs and coordinating with community resources.
Review clinical documentation and ensure compliance with hospital and regulatory standards.
Provide education and counseling to patients and families regarding treatment plans and available support services.
Monitor quality indicators and participate in continuous process improvement initiatives.

Key Requirements and Technology Experience:

Key Skills; BSN, BLS, experience as a Registered Nurse.
Current Registered Nurse (RN) license in the State of Georgia.
BSN strongly preferred.
2+ years of recent clinical nursing experience in an acute care or case management setting preferred.
Strong communication, critical thinking, and organizational skills.
Knowledge of care coordination, discharge planning, and community resources.
